Weekend Team Care Co-ordinator

We have three locations across Wiltshire (in Chippenham, Royal Wootton Bassett and Trowbridge) which allows us to provide care and support across the Wiltshire Area.

We are looking for somebody extra special to join our Weekend Office team as a Care Co-ordinator. The weekend team supports all three of our offices by enabling us to have a full staff outside of usual working hours and the role includes working Friday through to Monday each week from our Chippenham Office.

The role is primarily office based but will include covering care calls as and when needed and helping with on call.

Duties that will be included:

• All administration which will involve: Reviews, Spot checks, Care Plans and Auditing.
• Receiving and making calls to clients and carers.
• Take details for new referrals.
• Managing and planning staff and client rotas.
• Dealing with medication changes.
• As and when needed you will be required to cover care calls out in the community.
• You will be part of a team of 3 and will help and support each other including on call.

The salary for this role is £24,000 PA and is based on a four day working week (Friday through to Monday) – not Pro Rata! In addition, we offer a number of great perks working with us, including
